Citrullinated peptide antibodies (CCPAb) are produced by the immune system as a response to rheumatoid arthritis. Citrullinated peptides participate in the inflammation related to rheumatoid arthritis and are created on the synovial membranes of those suffering from rheumatoid arthritis. They are released into the blood, which helps determine their concentration.

When should citrullinated peptide be measured?

The test is usually carried out when examining ambiguous joint symptoms or suspecting rheumatoid arthritis.

Rheumatoid arthritis includes the following symptoms:

  • Painful, swollen joints in the hands and wrists
  • Pain sometimes affecting the elbows, neck, shoulders, hips, knees, and/or feet
  • Stiffness of the joints in the morning, which eases towards the evening
  • Fatigue
  • Fever
  • Lumps under the skin, particularly in the elbows
  • General feeling of malaise
What does a citrullinated peptide test measure?

The test measures the blood for citrullinated peptide antibodies. The test is used to diagnose rheumatoid arthritis. The accuracy of the test is 96–98%, making it a precise method for diagnosing rheumatoid arthritis.

However, the test cannot be used to monitor rheumatoid arthritis.

Normally, the result is:

Reference values:

  • < 7 U/ml: negative
  • 7–10 U/ml: weak positive
  • 10 U/ml: positive

The higher the values, the poorer the prognosis for the rheumatoid arthritis and the more efficient treatment is required.

The CCPAb levels are usually elevated in connection with rheumatoid arthritis. Rheumatoid arthritis is an autoimmune illness that causes changes, pain, and stiffness in the joints. Rheumatoid arthritis usually develops at the age of 40–60. Rheumatoid arthritis may develop quickly or slowly, become symptomless or even go away.

The value cannot be too low, as the result is either positive or negative. Usually, the citrullinated peptide value is negative.
